Our dedicated team of social workers, Case managers, care coordinators, counselors, and community health support staff are passionate about helping others. We are experienced and resourceful professionals who are committed to making a positive impact in our community.
Our Programs
We offer a wide range of programs to meet the diverse needs of our clients, including program coordinated counseling (group & individual), referral managements, case management for chronic diseases.
We also work with Medicaid and Medicare Managed Care Organizations to provide Health Homes Program deliverables.
We work with FeedAmerica#FoodLifeLine of WA to help overcome hunger by distributing culturally appropriate foods to those in need around our community.
All our programs are designed to provide the support and resources necessary for our clients to achieve their optimum health goals.
