We help facilitate health promotion, advocacy, and case management, care coordination, community Integration, Literacy, ambulatory medical & social transportation and interpretation service.

Our missions are central to our efforts in promoting individualized care to all. As our social responsibility to our community, we have closely identified ourselves with work around refugees and immigrants from many walks of life.

Some of the crisis areas we work on with clients include but are not limited to Housing, Medical Access, Mentorship, Mental Health, Addiction and Chronic diseases.

With Over a decade of different experiences bridging barriers and working to improve and advocate for better healthcare systems.

Our health & social team will assess you and help you create a custom recovery plan (ISP) that's right for you including your basic needs, short term and long-term goals.

We understand the importance of educating and informing you on the most effective ways to take care of your body, so that you have the right information before you decide.

Our experienced non-medical professionals CHW's put your healing needs first. We are proud to provide a high-quality level of customer service, medical experience, and commitment to health and wellness to all our patients.

Our goal is to make sure you make an informed decision for your healthcare treatment and services.

Our Services

Access to Basic Needs; Food Security & Wellness

We have been distributing organic produce and gift vouchers since the pandemic hit, most of our clients were affected by the shut-down and as a result their wellness took a dive to the worst. In efforts to keep our vow and social responsibility around wellness disparities, we embarked on shopping and linking with local farmers to put together produce boxes and hygiene supplies for the clients. We were able to serve 45households every Wed, Thru and Friday.

In July 2022, South King Healthcare Services in Partnership with Feed America#Foodline program in WA, started receiving more fresh and non-perishable goods for our clients. This is a partnership that Feed America-Foodline has with farmers in the pacific Northwest and Donors who help support feed their community members.

With these produces, we deliver fresh organic produce to our clients who have limited access and affordability to healthy options.

Through this extended support from Foodline, we are able to support 100 clients every week as well as helping them navigate health and social services that help them achieve their Health outcomes.

South King Healthcare serves client with prism level of 1.5 and above, who have at least one chronic condition, and Must be Medicaid eligible.
Core services provided in this program are:

Comprehensive care management, care coordination, comprehensive care transitions, health promotion, individual& family support & referral capacity and social support services in the community.

OUR INTEREST IN FATHERS Working with fathers Since 2020, approximately 3.5 years now, we have collectively served about 65 young men in the age groups of 17-38yrs of age. -Application for work first programs with Dshs/local workforces. -Any public Services including insurance renewals (EBT,Phone,Health Coverages) -Scheduling counseling and Psycho-sexual therapy.-Community Intergration after incarceration -Probation volunteer Opportunity to serve community and build good conduct record. -Connecting clients to community engagement services for clients with repeat offenses that are trying to re-establish their steps in the community. -Access to mental health and continuation of care-Coordinating clients medical and transportation needs. -Advocating for client's rights for services

Our CHW team who are pro-efficient in different Language will also work to maximize your prevention strategies. We strive to help you improve your quality of life, achieve your wellness goals, and support your best possible life. We are available to you when you need us, for basic needs as well as achieving your health goals.

South King Healthcare Services as a diverse community organization that is making a difference in our communities, loves to engage and integrate with each other.

These are clients only meetings groups of at-least 6-8 with their consent, they meet and have a member of the community leadership come and help them navigate services around them.

They also get education and information regarding different opportunities of care available for them.

With assets of legal understanding, we are able to guide and educate Asylee's, Immigrants and refugees to service, legal help and basic family immigration law.SKHS has a DOJ-BIA educated community legal advocate in the process of becoming accredited who can provide non- legal guidance and education.

We provide a variety of occupational health services to help businesses maintain a healthy workforce. Our services include pre-employment screenings, drug testing, and injury management.

Past Engagements & Work

Care coordination on chronic diseases in Pierce County. YR 2022-2023

Promoting prevention care against pandemic- Covid 19 discussions. YR 2020-2022

In partnership with Elevate Health of Pierce County (Accountable Communities of Health) We extended the more needed services of complex case management to the clients with chronic diseases in the county, scheduling appointments, medical care coordination, scheduling transportation and assisting with referral services. South King Healthcare Services is part of the Power of Provider Washington Network that is helping support, educate, advocate and communicate to clients about vaccines and their use.

We are bridging the medical vaccine accessibility barriers for people of color and those who are unable to express their needs.

We help facilitate the appointments for client in partnership with king county vaccination sites.

South King Healthcare Services is one of 49 social and health organizations selected that received funding from the Connect2 Community Network Catalyst Fund.

Qualifying organizations provided services in communities that are hard-to-reach or experience health disparities, and they served a significant portion of Medicaid, low income and/or under- or un-insured community members in King County.

The awards provided support to enable recipient organizations to join and engage with the Connect2 Community Network through its first technology partner, Unite Us, a web-based platform that enables closed-loop, bi-directional referrals.

SKHS was awarded a curriculum teaching opportunity as part of continued community Engagement and education on post-Covid-19 conversations on immunization and Vaccines.

In Person sessions were held in March 2024orkers at SKHS became Vaccination Community Navigators (VCN) by learning how to encourage vaccination in their communities and navigate people to vaccination services.

We provide comprehensive care for seniors, including geriatric assessments, medication management, ambulatory wheelchair services.

Our team is dedicated to helping seniors maintain their independence and quality of life and most important their mobility around accessing services far from home in a timely and safe manner.
